创建判定 Decisions

OpenRouter Decisions API 兼容接口，用于 typesafe/jev-* 系列判定模型（如 jev-1.13）：给定 state（待判定的状态/文本）和 questions（若干个 noul / choice / score 类型的问题），模型返回结构化 answers。请求体与 OpenRouter 原生格式逐字节一致地透传，仅 model 会按渠道映射改写。

约束：model、state、questions 必填；每个 question 需要 type（noul | choice | score）与 instructions，choice/score 还需要非空 criteria；不支持流式（stream 只能省略或为 false）。jev 系列模型只能在本接口调用，在 /v1/chat/completions 会返回 400 unsupported_endpoint_for_model。

计费按响应 usage.input_tokens / output_tokens 乘本站价格；响应中的 usage.cost 是供应商成本字段，不是本站扣费金额。

代码事实源：router/relay-router.go（decisionsRouter.POST("/decisions")）、relay/decisions_handler.go、dto/decisions.go、docs/decisions-api.md。

模型名：本站对外模型名为 jev-1.13（也接受 jev-1.13）。响应中的 model 字段是上游返回的完整版本名。

最后校验/修改：2026-09-28（API Base URL 统一为 https://api.crazyrouter.com，示例模型名以 GET /v1/models 返回为准）；实测：stream=true / 缺 questions 均返回 400 invalid_request_params；jev 模型在 /v1/chat/completions 返回 400 unsupported_endpoint_for_model；api.crazyrouter.com 已放行本路径
